I, the student, acknowledge that I am responsible for my actions during all youth ministry meetings, events, and trips under the sponsorship and guidance of Bozeman United Methodist Church. By signing this covenant, I agree to do no harm, to do good, and to uphold a safe, encouraging, and inclusive atmosphere. I agree to respect my peers and leaders, to refrain from physically harming others or using derogatory, vulgar, or otherwise harmful language, to respect church property or the property of any other place we visit, and to adhere to any additional rules, guidelines, or instructions set forth by my leaders.
While my leaders promise to approach correction with empathy, and the consequences for breaking these guidelines will always be done with the goal of reconciliation to give me the opportunity to learn and grow, I understand that should a disruptive and/or harmful behavior persist or escalate beyond control, my parent or guardian will be contacted to come escort me from the premises.
Liability Release & Consent to Medical Treatment
I, parent/guardian of the above registered student, give the above named student permission to accompany their youth leaders around the Bozeman UMC Campus and neighborhood for activities related to Youth Ministry. I understand that while on the property, my student will be covered by that church's liability insurance if something were to go wrong with the building or equipment that belongs to that building. I understand that my student will be covered by my own health insurance plan if they become sick or injured on Bozeman UMC's premises not due to building or equipment failure. I further expressly authorize and consent to any medical care by qualified medical personnel for my student, if necessary, because of illness or injury.
Bozeman UMC (and its staff) would like to take and share photos of church activities and its church family. If you would like your student not to be featured in photos, you may opt out below.
